    The importance of the endoscopic evaluation in inflammatory bowel disease (IBD) management has been recognized for many years. However, the modalities for reporting endoscopic activity represent an ongoing challenge. To address this, several endoscopic scores have been proposed. Very few have been properly validated, and the use of such tools remains sub-optimal and is mainly restricted to clinical trials. In recent years, a growing emphasis of the concept of 'mucosal healing' as a prognostic marker and therapeutic goal has increased the need for a more accurate definition of endoscopic activity in both ulcerative colitis (UC) and Crohn's Disease (CD). In the present review, the evolution of the challenges related to endoscopic scores in IBD has been analyzed, with particular attention paid to the renewed relevance of endoscopic activity in recent years. Currently, despite the growing relevance of endoscopic activity, evaluating this activity in IBD is still a challenge.     From 2004 the Frascati Beam Test Facility (BTF) in the DAFNE accelerator complex provides to the external user up to 1E10 electrons per bunch or up to 10E9 positrons per bunch to develop their detectors.     Optical coherence transfer mediated by free electrons

    We theoretically investigate the quantum-coherence properties of the cathodoluminescence (CL) emission produced by a temporally modulated electron beam. Specifically, we consider the quantum-optical correlations of CL produced by electrons that are previously shaped by a laser field. Our main prediction is the presence of phase correlations between the emitted CL field and the electron-modulating laser, even though the emission intensity and spectral profile are independent of the electron state. In addition, the coherence of the CL field extends to harmonics of the laser frequency. Since electron beams can be focused to below 1 Ă…, their ability to transfer optical coherence could enable the ultra-precise excitation, manipulation, and spectrally resolved probing of nanoscale quantum systems

    OBJECTIVE: Ulcerative Colitis (UC) is a chronic inflammatory disease of the colon of unknown etiology. Several clinical indexes have been proposed for UC disease activity evaluation, but none have been properly validated. Moreover, the reference parameter for the scores and their prognostic value is not clear. Mucosal healing has been recently proposed as an important end-point. Aim of the present study was to evaluate the correlation of four clinical indexes with objective diagnostic tools for UC evaluation, the discriminative ability in identifying patients with endoscopic mucosal healing, and to analyze the possible prognostic indication for disease course in 1 year of follow-up. PATIENTS AND METHODS: We analyzed data of 75 patients recorded in regular follow-up visit in IBD clinic at S. Andrea Hospital, Rome, between 2007-2011. We recorded clinical data and lab tests at the time of the visit, and endoscopic/ histological repor ts performed within 1 month. Clinical indexes (Seo' activity index, Simple Clinical Colitis Activity Index, partial Mayo score and Endoscopic-Clinical Correlation Index) were calculated and correlation to endoscopic and histologic activity, and to C-reactive protein increment, was assessed by mean of Spearman's rank correlation. Discriminative ability of the indexes for patients with and without endoscopic mucosal healing was tested by calculation of area under ROC curve (AUC). Patients with low and high clinical scores were compared for number of flares and increment of therapy during 1 year of follow-up. RESULTS: Clinical indexes had a good correlation with endoscopic activity (mean r = 0.73 ± 0.06), a fair correlation with CRP-increment (mean r = 0.55 ± 0.01) and a poor one with histologic activity (mean r = 0.35 ± 0.01). The discriminatory ability of the indexes for endoscopic mucosal healing was good for all the indexes (mean AUC = 0.87 ± 0.05). Patients with high clinical score had more flares and required more frequently increase of therapy at 1 year of follow up compared with patients with low score. CONCLUSIONS: Clinical indexes have a good correlation with endoscopic activity and can discriminate patients with and without mucosal healing. Patients with low and high score have different risk of disease flare and of need to increase therapy at 1 year. Clinical indexes may represent a useful tool for disease assessment in clinical practice in UC outpatients with mildmoderate disease

    Transition to parenthood is a critical stage of life due to several changes the couple has to handle. A large body of studies described how transition to parenthood can be linked to the onset of depressive symptoms, as well as the perception of a low social support, and an increased stress, representing a risk for the early mother—baby relationship. Infant Massage (IM) emerged as an helpful tool to improve maternal skills in interacting with the baby, and leading toward a decreasing of post partum symptoms. However, a growing body of literature highlights that men also may experience post-partum diseases, representing an additional risk for the development of the baby. To date, no study observed the impact of the infant massage on both partners. The aim of the current qualitative research is to observe the impact of the IM on a single couple of parents at childbirth. Pre (Time 1) and post-intervention (Time 3) procedure has been established to observe the changes occurring over the time in the couple. In particular, each member of the couple filled out the EPDS, the BDI-II, the MSPSS and the PSI-SF both at Time 1 and at Time 3. The treatment (Time 2) was represented by the IM training, and lasted 4 weeks. Findings revealed a decrease in depressive symptoms in both partners, as well as an improvement of their perception of stress related to parental role. No changes has been detected with respect to the perception of social support. The IM seems to be a helpful approach to prevent the establishment of pathological conditions in new parents. Although no direct measures on the child were used, the current qualitative data seem to suggest that the IM may represent a valuable tool to prevent the onset of early negative outcomes of the baby. Further investigations and empirical data are needed to improve the knowledge in this field

    Nanoscale Nonlinear Spectroscopy with Electron Beams

    We theoretically demonstrate the ability of electron beams to probe the nonlinear photonic response with nanometer spatial resolution, well beyond the capabilities of existing optical techniques. Although the interaction of electron beams with photonic modes is generally weak, the use of optical pumping produces stimulated electron–light interactions that can exceed order-unity probabilities in photon-induded near field electron microscopy (PINEM). Here, we demonstrate that PINEM can locally and quantitatively probe the nonlinear optical response. Specifically, we predict a dependence of PINEM electron spectra on the sample nonlinearity that can reveal the second-harmonic (SH) response of optical materials with nanometer resolution, observed through asymmetries between electron energy losses and gains. We illustrate this concept by showing that PINEM spectra are sensitive to the SH near field of centrosymmetric structures and by finding substantial spectral asymmetries in geometries for which the linear interaction is reduced

    Issues in Choosing the References to Use for Spectral Ratios from Observations and Modeling at Cavola Landslide in Northern Italy

    A reference site has to be free of amplification or de-amplification effects, namely with no troughs and peaks in its Fourier amplitude spectrum. At the Cavola landslide we show that this spectrum is dependent on the direction of propagation of wave fronts for incidence angles in the range 30° to 90°. Our study is based on comparison of spectral ratios from observations and 2D numerical simulations. We have modeled propagation in a 2D profile for SH and SV waves with several incidence angles in the 0° ±90° range, where 0° and 90° are respectively vertical and horizontal incidence, except that ±90° denotes Rayleigh waves in the P-SV. We discuss in detail the result for angles of incidence of 0, ±20°, ±60°, ±90°. We have obtained observed horizontal-to-horizontal earthquakes spectral ratios using three reference sites. Two of these have matching receivers in the model, located at the opposite ends of the 2D profile. Overall observations are matched best when the reference site is located on the same side of the landslide as the incoming wave front. We also find general agreement of the observed H/H spectral ratios from earthquakes with H/V and H/H spectral ratios from noise, and the match between H/V values from noise and synthetic spectral ratios using an absolute, flat half-space reference is very good. On the other hand, 1D modeling performs poorly in comparison with 2D modeling in our case, for which the shape ratio h/D = 0.2 is intermediate between primarily-1D and strongly-2D wave propagation according to the classification of Bard and Bouchon (1985)
